The above named Life Skills Coach is applying to become a
Mentor for the Saskatchewan Life Skills Association. The prospective Mentor must meet the
following criteria and provide the appropriate documentation with this form.
CRITERIA:
- Must be a current member of SLSA
- Minimum of 3 years experience as a Life Skills Coach
- Minimum of 1000 hours of coaching in Life Skills groups
- Taken a Life Skills Coach Training course
- Letters of recommendation from two people who are familiar
with Life Skills and the applicant’s work
- Must be willing to enter the accreditation process in the
future
